What this is
When you start a visit at phoenixpharmacy.com/intake you tick one box that says you agree to these terms. This page is the full text of what that box means. We record the date, time, and version you agreed to.
1. Video visit with a pharmacist
You consent to a consultation with a licensed Phoenix Pharmacy pharmacist by live two-way video (telehealth), as permitted by California Business and Professions Code §2290.5. Video visits are not recorded. You may stop the visit at any time and ask to speak with a pharmacist by phone or in person at our La Palma store instead. The pharmacist may decline to dispense if a video consultation cannot be completed.
2. Receiving medication from the Phoenix Rx Kiosk
You consent to receive your prescription from the Phoenix Rx Kiosk, an automated patient dispensing system operated by Phoenix Pharmacy under California Business and Professions Code §4427 and following. A licensed pharmacist reviews every prescription before it is released and supervises the kiosk remotely. For a first fill of a new medication a pharmacist consultation is required before release. You may instead pick up at our La Palma store or use delivery.
3. Text messages and phone calls
You give Phoenix Pharmacy permission to contact you at the mobile number you provide about your prescription and your visit, by text message and by phone call, including automated, prerecorded, or AI-assisted messages and calls. This is not a condition of receiving pharmacy services. Message and data rates may apply. Message frequency varies with your prescription activity. Reply STOP to any text to end texts, or HELP for help. Text messages are not an encrypted channel; we keep them to the minimum, such as a link to your visit or a pickup notice.
4. Artificial intelligence
You agree that Phoenix Pharmacy may use AI tools to read the insurance card you photograph, to help our staff prepare your case, and, where you have agreed to be contacted, to assist with routine communications. AI never makes a clinical decision, never dispenses medication, and never replaces the pharmacist. You confirm every value read from your card before it is used. AI vendors that process your information are business associates required to protect it and not to use it for their own purposes.
5. Privacy notice
You acknowledge that you have been given access to our HIPAA Notice of Privacy Practices, which explains how we use and protect your health information and the rights you have over it.
6. Changing your mind
You can withdraw any of these consents at any time by contacting our Privacy Officer at privacy@phoenixpharmacy.com or (714) 690-0349, or by replying STOP to a text. Withdrawing consent does not affect your right to use the pharmacy; it may mean we contact you a different way or ask for information again.
